Reappointment Information

After the initial one-year appointment, clinical faculty appointments may be renewed for up to three years, contingent upon continued fulfillment of the criteria for appointment as assessed by the Clinical Faculty Personnel Committee (CFPC).

For the renewal of a UH clinical faculty appointment, the following materials are needed for reapplication:

  • Fill out the application form and identify the following over the past THREE years:
    • Details on teaching activies that you have particiapted in. 
    • Any awards received.
    • Recent abstracts, reports, manuscripts, and publications.
    • Details on non-teaching JABSOM activities. 
    • Community Service
  • An updated CV (within 1 year).
  • Evaluations of your teaching (this will be pulled by the department once you initiated the reappointment process). 

Please be advised that, in order to be considered for renewal, you must commit to dedicating 60 hours each calendar year to formal teaching activities with UH JABSOM, UH Residency, or UH Fellowships. Other Materials Required for Renewal

As required by the UH Board of Regents, all Clinical Faculty members must complete the Title IX training every two years.

How to complete the training:

  • Please use your UH username and password to access the training. If you do not have a UH account or have forgotten your credentials, click here.
  • You can access the training by clicking here.
  • For detailed instructions on how to complete the training, refer to the guide.
  • Please ensure you complete the training titled "Preventing Harassment & Discrimination: Gateway (Full Course)".
  • The training takes approximately one hour. However, you can complete it in intervals

Technical Tips:

  • Use a desktop or laptop with the latest version of Google Chrome for the best experience.
  • Clear all open tabs before starting the training.

State law requires clinical faculty to sign an ethics acknowledgment form every four years. While clinical faculty members are not required to complete the official State Ethics Training due to their non-compensated status, it is still important that you familiarize yourself with the State Ethics Code.
